CARE TEAM

Our Care Management team are specially trained to work alongside the physicians and participate in your child’s care. They help identify needs, participate in setting goals to improve your child’s health, assist with medication management, ensure patients and families understand your child’s plan of care, help provide resources and follow up with patients after a visit to the urgent care, emergency room or hospital admission.

Sheri T.  LPN, AE-C
Sheri T.  LPN, AE-C

Sheri received her LPN degree in 1997 and has been working in the pediatric specialty for her entire nursing career.  She joined Cascade Pediatrics in 2006.  Sheri became a certified asthma educator through the National Asthma Education Certification Board in 2013.  In 2016, Sheri received her care management certification through the MI Center in Clinical Systems Improvement (MI-CCSI).  She has a passion for educating patients on self-management of their asthma symptoms as well as monitoring their progress.  

 

Sheri enjoys spending time with family, gardening, working on crafts and being outdoors. 

Michelle B., BSN, RN
Michelle B. BSN, RN

Michelle earned her Bachelor of Science in Nursing degree in 2001 from a joint program at Bowling Green State University in Ohio and also the University of Toledo Medical Center.   She joined Cascade Pediatrics as a RN-Care Manager in 2018.  Michelle received her care management certification through the MI Center in Clinical Systems Improvement (MI-CCSI). In February 2021, she earned her certification in the Collaborative Care Management (CoCM) Program through MI-CCSI. This program allows her to manage patient care and collaborate with Psychiatry. Michelle has a passion for patient education in helping others acquire appropriate care for their mental health.

 

Michelle enjoys reading, spending time with her family, being outdoors, walking, gardening and cooking.

Sue D. RN

Sue received her nursing degree from Mott Community College and has been a registered nurse for over 22 years. She began her career at McLaren Trauma Center in Flint, MI as a trauma nurse before joining Owosso Memorial Hospital where she stayed for 21 years.  Sue has worked in many different specialties throughout her career including Labor and Delivery, Surgery and Gastroenterology. Sue is also a veteran of Desert Storm serving in the Air Force/MI Air National Guard for 10 years.  

 

Sue joined Cascade Pediatrics as a Behavioral Health Care Manager in 2022. She received her care management certification through the MI Center in Clinical Systems Improvement (MI-CCSI).  

 

Sue has one daughter, Isabella, who is a teacher.  In her spare time, Sue enjoys pickleball, working out and traveling.

Nutrition Support

Registered dietitians who work with the physicians to help guide and support patients and families with various issues related to nutrition

Lactation Support

Certified lactation specialist who works with the physicians to help new moms with breastfeeding.
